The Utah Transit Authority (Authority), Salt Lake City, Utah, is requesting proposals (RFP) from responsive, responsible persons or firms to furnish tires and tires services on a lease basis. All correspondence, questions of any kind, and requests for a proposal package concerning this RFP which is in the form of an e-mail, shall be directed to Grants and Contracts Administrator, Ms. Jolene Higgins, UTA, 669 West 200 South, Salt Lake City, Utah 84101. Email: jhiggins@rideuta.com. If requesting the procurement package, please provide your complete e-mail address and a land line phone number. Ms. Higgins is the only authorized contact during the procurement phase. Prospective participants are solely responsible for timely delivery of any question or request. Any written question or request must be received by UTA's Grants and Contracts Administrator no later than 2pm MDT on Monday, June 10, 2013. Questions received after the deadline may not be honored. Sealed proposals marked: RFP UT-13-018JH Bus Tire Lease Services must be received at or before 2:00 p.m. MDT on Thursday, June 20, 2013. Any proposal received thereafter shall be considered nonresponsive. Please note that sealed proposals are to be received at UTA offices at 669 West 200 South, SLC, UT 84101 ONLY. It is the responsibility of the Proposer to assure that its proposal arrives at the designated location by the specified time stated above. This project will be financed with financial assistance from the Federal Transit Administration (FTA) of the United States Department of Transportation. The contract awarded will be subject to the financial assistance contract between the Authority and the U.S. Department of Transportation. All offers will be required to sign the following certifications, Affirmative Action and Equal Employment Opportunity Statement, Buy America Certifications, Certification regarding Debarment and Suspension, Certification of Restrictions on Lobbying, and Cargo Preference-Use of United States Flag Vessels. The Contractor will be required to comply with all applicable and equal employment laws and regulations. The Utah Transit Authority in accordance with Title VI of the Civil Rights Act of 1964, as amended, and 49 CFR Part 26, will afford Disadvantaged Business Enterprises (DBE's) full opportunity to respond and will not discriminate against any interested firm or person on the basis of race, color, sex or national origin in the review of qualifications or contract award. Responsive, responsible firms will be required to address the utilization of DBE's in their proposal documents submitted. The Authority has assigned a Race Neutral DBE goal for this project. Any name appearing on the Comptroller General's List of Ineligible Contractors for Federally Financed and Assisted Construction, Materials or Equipment Contracts is not an eligible proposer. By requesting and accepting this procurement package, UTA is not obligated to award any contract or to pay any costs incurred in preparation of a response. UTA reserves the right to waive any minor irregularities and/or informalities, to reject any and all proposals for sound business reasons, to re-advertise and to award contracts in the best interest of UTA. No oral, telephone, telegraphic, Email, or facsimile proposals or modifications will be considered.

2013 LIFT STATION PROJECT

Ashley Valley Water and Sewer Improvement District

Sealed bids for 2013 LIFT STATION PROJECT will be received by the Owner, ASHLEY VALLEY WATER AND SEWER IMPROVEMENT DISTRICT, at its office located at 1344 West Highway 40, Vernal Utah until 10:00 am, local time, on May 21, 2013. Bids sent by mail shall be addressed to the ASHLEY VALLEY WATER AND SEWER IMPROVEMENT DISTRICT, Attn: Dave Hatch, P.O. Box 967, Vernal Utah 84078, and shall be labeled "Bid for 2013 LIFT STATION PROJECT". The project is a remodel and addition to the Ashley Lift Station, a sewer pumping station. Replacement of two pumps and various piping are included in the project. Excavation around the existing wet will will be performed and a concrete addition to the existing wet well will be constructed. Additional piping will be installed from the building addition to the existing force main. Several valves, elbows and tees will be installed in the piping. The Project is located approximately 170 miles east of Salt Lake City, Utah. Copies of the Project Manual, including the Bidding Requirements and proposed Contract Documents may be obtained at the office of Engineering Services, Inc, 2028 West 500 North, (P.O. Box 1485), Vernal, Utah 84078, Telephone (435) 781-2550, upon payment of $50.00 for each set. Return of the manuals is not required, and the amount paid for the manual is nonrefundable. Persons wishing to obtain copies of the manuals shall furnish their street and mailing addresses for delivery of documents and addenda. Each bid shall be correctly prepared and submitted on the prescribed Bid Form with required attachments, and shall be accompanied by a Bid Security as prescribed in the Instructions to Bidders payable to the Owner in an amount not less than five percent (5%) of the amount of the bid. The Successful Bidder will be required to furnish Performance and Payment Bonds, each in an amount not less than one hundred percent (100%) of the full contract price. A mandatory pre-bid conference will be held at 10:00 am local time, on May 16, 2013 , at the offices of Ashley Valley Water and Sewer Improvement District located at 1344 West Highway 40, Vernal, Utah 84078. The Owner reserves the right to reject any or all bids and to determine which bid is, in the Owner's judgment, the lowest responsive bid of a bidder or group of bidders. The Owner also reserves the right to waive any informalities in any bid and to delete certain items listed in the bid as set forth therein. Bids received after the time established for receiving bids will not be considered. Except as provided in the Instructions to Bidders, no bidder may withdraw his bid after the time established for receiving bids or before the award and execution of the contract, unless the award is delayed for a period exceeding sixty (60) calendar days. Bidders must be licensed in the State of Utah for an amount equal to or greater than the amount of their bid and for the type of work required, at the time of bidding and throughout the period of the contract.
